We begin with a structured audit of recent documentation across your program or organization, from intake records through individual service notes. This review identifies staff documentation patterns, system-wide trends, workflow gaps, and Medicaid compliance risks.

You'll receive a clear, organized report outlining what's working, what needs correction, and which areas require immediate attention to reduce audit risk, prevent findings or clawbacks, and strengthen documentation defensibility.

Each month, we review a rolling 30 day documentation sample to ensure prior issues are being corrected and documentation remains compliant with Medicaid requirements. These routine reviews help catch recurring errors, support new staff, and reinforce consistent documentation habits over time.
